Outstanding Contribution to Community


This award is to recognise a health professional or practice that has made an outstanding contribution to primary health service delivery and acknowledges a remarkable accomplishment in the unfolding story of Primary Health service delivery in the Murrumbidgee.

This award is to recognise that an individual practitioner or practice has engaged deeply with the community, understands the characteristics and needs of the community, and has worked tirelessly to deliver services to the community within which they operate.

It is to recognise the work of those professionals in our community who have gone above and beyond.

Criteria

  • Ensure the focus of the application is on the individual practitioner or practice rather than a particular program

  • This award is peer nomination only and not self-nomination

  • Evidence that the person has made a significant and extensive contribution Primary Health service delivery in the Murrumbidgee

  • Evidence that the individual practitioner or practice has a history of being innovative, maintaining a high standard of service and supporting the Murrumbidgee community
